In this episode, we discuss Chronic Care Management (CCM).  We take the opportunity to introduce many of our listeners to this billable service for which Medicare pays that many providers may not be doing just yet.  CCM was introduced by Medicare back in 2015 and has just now become a topic that organizations are interested in. We break down what CCM is, the requirements for it, and why, most importantly, it benefits the patient.
